Transportation Permission Form

The transportation of students in private non-commercial vehicles by someone other than a staff member for a school sponsored activity (Field trips, athletic events, band festivals, etc.) is prohibited unless prior approval in writing is granted by the school principal to a parent/guardian. 

The student must bring a note to the office prior to the event so that it can be stamped and approved. The approved note should be presented to the coach prior to departure. Please be advised that parents/guardians are only permitted to transport their own child.
